Brooks St. Group Home
Since 1989, Brooks Street Home has been an integral part of Mid-Valley, providing safe and friendly services to all 5 residents who reside within the home. Here at Brooks Street Home are dedicated and friendly staff who work with each resident on an individual 
basis to help them learn and grow in a safe family-like environment. We pride ourselves in giving each resident the best opportunity to live a fully supported life. Our goal is to fully support the best quality of life to each resident. Our motto is: “May each guest who enters our home leave with fond memories and new friends.”
Valley's Edge Group Homes
Valley’s Edge consists of two 5 bedroom homes in the west side of McMinnville. The homes were built and customized in 2009 to meet the needs of residents who have high medical needs. Each home is equipped with ‘walk in’ showers, extra-wide doors, whirlpool tubs, covered patios, and state of the art kitchens. Each individual has their own personalized private bedroom and there is 24 hour care by staff who enjoy assisting the residents with learning, improving and retaining their daily living skills.
Cherry Heights Group Home
Cherry Heights opened in May of 2009 and three wonderful young ladies, all having grown up together in Dallas, Oregon, moved in.
Cherry Heights has one full time program manager and eleven full time female staff who present a dynamic team of energy, expertise, and experience. We base our team success on operating and maintaining a fun, family-oriented environment.
We enjoy activities such as visiting local vendors, going for walks and visiting parks, going to the waterfront carousel in Salem, going to the movies, and slumber parties.
Davis St. Apartments
The Davis Street Apartments, which opened in April 2001, is a "tenant support" program for 9 residents. Tenant support means that the residents have the independence of living in their own apartments but also have the security of 24-hour staff support on the premises. Located at 5th and Davis Streets in McMinnville, this program is one of only a handful of its kind in the state.
Baker St. Apartments / Supported Living Services
Currently Mid-Valley serves 19 clients out of the Baker St. Supported Living Program. 10 of our clients live at the Baker St. Apts, and the rest live in their own apartments in the McMinnville Community. We have 3 full time Support Staff and one half time Support Staff. We also have 4 Support Staff who work as 1:1 staff for 2 of our clients, who need more supervision. In addition we have a part time Activity Director who provides activities on Wednesday, Friday and Saturday, which usually lasts about 4 hours. This includes activities at the Baker St. office, activities out in the community and dinners every other Wednesday night. Many of our clients have jobs during the week, and they really enjoy the activities, and the social interactions. Support staff provide services to our clients which includes taking them grocery shopping, 
personal shopping, assisting clients making healthy choices, banking, medical appointments, medication management, vacation planning and transportation if needed. Assistance is also provided filling out applications needed for re-certifications. Staff do regular safety and medication checks, for all clients who self administer their own medications, and if there are ever any issues the Program Manager is notified immediately. All SLP clients are provided with a posted phone number for the SLP emergency phone for 24 hour access, if needed. We constantly encourage our clients to be as independent as possible, but are there to support them whenever needed.
